For most families – us included – getting a date night out of the house is a rare treat. The last thing you want to worry about is remembering and writing down all the important details about your kids.

Babysitter information
This printable babysitter information sheet is simple. It includes categories and lines to fill in. The categories ensure you don’t forget an important detail (like your cell phone number!).
They’re simple enough that you can fill them in with your info. I know one size rarely fits all, so I wanted to make this something that most parents could use.
I included space for up to 4 kids, so if you have more, you can use 2 sheets. Two forms print on a single page, so you can print and cut down the middle. And they don’t use a ton of ink.
Each form includes space for:
- Your name
- Where you’re going
- When you’ll be home
- Home address and phone number
- Emergency contact info
- Important rules and reminders
- Your child’s schedule
- Each child’s name, age, allergies, and special notes
I recommend printing off several pages, cutting them down the middle, and stacking them somewhere (like the mail organizing station by the door) so that you always have one handy.
